Instructions:

• Registration deadline is October 20, 2006.
• All seats are $40 each. Tickets will be held at the door.
• Please provide your contact information and the names of your guests. This information is for our use only and will not be shared with others.
• If you are also attending the dinner, indicate yourself as Guest 1.
• If you are donating seats to students, indicate "Donate" in appropriate number of boxes.
• When you press "Submit Form" at the bottom of this page, you will be taken to the secure online payment page.

Example: If you are purchasing two individual dinner seats and donating two seats to students, you will purchase four individual dinner seats for a total of $160.
