• Registration deadline is October 24, 2010.
• Please provide your contact information and the names of your guests. (This information is for our use only and will not be shared with others.)
• If you are also attending the dinner, indicate yourself as Guest 1.
• If you are donating seats to students, indicate "Donate" in appropriate number of boxes.
• All seats are $50 each.

  • Example: If you are purchasing two individual dinner seats and donating two seats to students, you will purchase four individual dinner seats at $50 each for a total of $200.

• After you have typed in your information, use your browser's "Print" option to print this page. Mail it in along with your check made payable to UNA-GKC:
